What are the benefits of this Wellness Plan?

What does the membership include?
- The initial comprehensive exam, set of x-rays & routine cleaning**
- The patient’s periodic exam & routine cleaning at your next 6-month appointment
- Fluoride at every cleaning
- A 10% discount for ALL dental treatment including implants, veneers, whitening, occlusal guards/splints, cosmetic bonding, and clear aligner orthodontics
- Happy visits for children under 3 at no charge
- No annual limits
- This cannot be combined with any other dental benefit coverage
**If you require deep cleaning (Scaling & Root Planing or nonsurgical periodontal therapy) for gum disease, then note that the procedure is not included in the enrollment fee. However, you will be able to be enrolled in our periodontal plan and after your initial Scaling and Root Planing will have your four periodontal cleaning visits covered along with all the benefits above.
What is the cost?
- Individual Adult (18 and Older): $450 a year
- Individual Child (Under 18): $375 a year
- Periodontal Plan: $595 a year
- Additional Adult Family Member: $400 a year
- Additional Child Family Member: $350 a year
- You can cancel your membership in the Winter Dental Wellness Plan at any time, let us know in writing 30 days before you wish your coverage to end. Payments are non-refundable.
What forms of payments are accepted?
We accept cash, check, or credit cards. Membership is for 1 year beginning on the enrollment date, and membership dues are payable in full upon enrollment and are non-refundable.

Child Membership
Under 18

Periodontal Disease
$595/year
- Perio maintenance cleanings (up to 4 per year)
- Doctor exams (1-2 per year)
- Needed x-rays*
- Fluoride
- One emergency exam per year (if needed)
- 10% off all other treatments (some exclusions may apply)
- No Annual Limits
For patients enrolled in active periodontal maintenance due to prior treatment of periodontal (gum) disease.
Additional Family Adult Member
18 and Over
$400/year
- Professional dental cleanings (2 per year)
- Doctor exams (1-2 per year)
- Needed x-rays*
- Fluoride at every cleaning
- One emergency exam per year (if needed)
- 10% off ALL other treatments (some exclusions may apply)

Can I add members to my account?
Yes! You can add your entire family to your membership, those rates are listed above.

Can I use dental insurance with my membership?
Our dental wellness program is for uninsured patients or patients who choose not to utilize their insurance (for example, if they have out-of-network insurance). This offer is not valid in combination with dental insurance of any kind, so if you become insured or choose to utilize your insurance, your membership will be terminated. Dues are non-refundable.
